The Three-Phase Recovery Program

Comprised of a team that consists of psychologists, psychiatrists, social workers and intake CASAC personnel, the program is designed for people with moderate-to-severe alcohol and substance abuse problems. This program offers intensive initial support, followed by gradual step-down support as the individual moves into recovery.

 

• Primary care: one individual/family session per week, plus three extended groups three times a week, for four to six weeks

 

• Intermediate care: Individual/family sessions as needed, and two extended groups per week, for 12-15 weeks

 

• Continuing care: Individual/family sessions as needed, plus one group per week to address longer-term recovery issues

 

Fee:  Medicaid and Medicare accepted (Patients without insurance have access to a specialist who can help determine if they are eligible for Medicaid.)  Self-pay or commercial third-party clients accepted on a case-by-case basis.

The St. Vincent’s Hospital Manhattan Outpatient Chemical Dependence Program provides evaluation, group therapy and individual treatment to individuals of all ages, from adolescents to seniors, who suffer from dependence on alcohol or other drugs, including painkillers and methamphetamine. Treatment focuses on helping the client to establish and maintain sobriety. Psychiatric evaluations, treatment and medication management are provided when appropriate.

 

For individuals who require a more highly structured program in order to prevent inpatient hospitalization or to provide transitional structure after inpatient rehabilitation, St. Vincent’s offers an intensive outpatient program (IOP), with treatment offered both in English and in Spanish. The program provides group and individual clinical services from nine to 15 hours per week. After completing the intensive program, clients may move into the outpatient program. We also offer evening program groups Monday through Friday.

 

 

The Outpatient Chemical Dependence Program offers the following specialty services:

 

Methamphetamine (Crystal Meth) Treatment Track

This newly developed track treats methamphetamine (crystal meth) users, focusing on the impact of an individual’s drug use and risky behaviors in a context of recovery readiness counseling, harm reduction, support and eventual abstinence.

 

Latino Program

The Latino program offers group and individual treatment in Spanish to monolingual as well as bilingual individuals. Groups focus on helping Spanish-speaking individuals to overcome problems with addiction in a culturally sensitive manner.

 

MICA Services

This program assists clients challenged with both mental illness and chemical dependency to reach sobriety while achieving and maximizing personal functioning. Treatment includes helping clients to explore the relationship between their sobriety and mental illness.

 

Older Adult Services

Groups in this track focus on addiction issues specific to older adults, including coping with retirement, changes in health status, and social isolation.

 

Adolescent Services

This after-school program is designed for clients ages 13 to 18 who are experiencing problems with alcohol and other substances. Family involvement is an integral part of the program.

 

Buprenorphine Treatment

Outpatient detoxification using buprenorphine is available for persons dependent on opiates such as pain medication, heroin or methadone. Buprenorphine has been shown to prevent withdrawal symptoms, reduce cravings and block the effects of opiates. Group therapy is an integral part of this treatment.

Safe Horizon offers services dealing with a myriad of issues including: domestic violence, rape/sexual assault, stalking, providing shelter, case management, counseling, and support groups.  Safe Horizon also offers other ancillary programs such as the Domestic Violence Accountability Program for batterers and the Project Safe program that provides free locks or cylinders for anyone that has filed a police report.  Safe Horizon houses psychiatrist, psychologists, and social workers on staff.

Counseling, Referrals and Support Services for Survivors

SAVI offers free and confidential counseling to survivors of rape, sexual assault, and incest. We also provide services to friends and families of survivors.

Counseling is important immediately after the assault. Counseling is also immensely valuable no matter how long ago the assault occured.

Counseling sites are located at The Mount Sinai Hospital, Elmhurst Hospital Center, and the Queens District Attorney's Office.

Free and Confidential Support Groups - Throughout the year, SAVI offers short-term groups for adult survivors of rape, incest, and childhood sexual abuse.

The New York City Gay and Lesbian Anti-Violence Project (AVP) is the nation's largest crime-victim service agency for the lesbian, gay, transgender, bisexual, and HIV-affected (LGTBH) communities. Since AVP's founding in 1980, we have assisted thousands of crime victims every year, from all five boroughs of the city.

 

AVP offers free and confidential support to victims of bias crimes, domestic violence, pick-up crimes, police misconduct, HIV-related violence, rape, and sexual assault. Individual and group counseling is available at AVP, while a bilingual (English/Spanish) crisis hotline offers 24-hour assistance from counselors and trained volunteers. In addition, AVP provides accompaniments and advocacy for our clients with the police, the courts, medical, and social service agencies.

 

AVP is the only agency in New York City that provides services specifically for LGTBH victims of domestic violence. We provide counseling, advocacy, and legal assistance to hundreds of survivors of domestic violence every year. We help people create safety plans and document the violence committed against them. In addition, AVP offers support groups for domestic violence victims, assists in obtaining orders of protection, and helps locate shelters and emergency housing options.

The Center offers short-term, directed treatment for adults who exhibit acute symptoms associated with posttraumatic and dissociative disorders and other trauma-based conditions. A national model in the treatment of adult posttraumatic syndromes, The Center’s innovative program provides rapid stabilization and essential training in self-management skills through the use of stage-oriented, cognitive and behavioral strategies. Designed to meet the individual needs of patients at different stages of treatment and respond to the challenges posed by a rapidly changing healthcare environment, our problem- and skill-focused continuum of care includes inpatient and partial hospitalization in the Washington, DC area.

The AOPC provides comprehensive psychiatric evaluations and symptom focused treatment to patients with acute, remitting episodes of illness, covering a wide range of Axis I and Axis II disorders.

 

Access Center 

All patients entering the AOPC receive a comprehensive psychiatric evaluation which includes both an extensive clinical interview and a battery of psychosocial assessment tests. These tests may include an abbreviated Structured Clinical Interview for DSM-IV (mini-SCID), Brief Psychiatric Rating Scale (BPRS), Beck Depression Scale and Beck Suicide Index. The initial evaluation generally takes about two hours and results in a plan for the patient's treatment. Following the evaluation, options are discussed with the patient and referral source to facilitate the patient's transition into treatment.

 

Treatment Programs

 A wide variety of treatments are available for both English- and Spanish-speaking patients with all major psychiatric disorders. Depending on the diagnosis and clinical needs of the patient the evaluation team recommends one or more specific forms of treatment. These include:

 

Psychopharmacologic Treatment 

Medication therapy may be the sole treatment for some patients, but it is usually offered in conjunction with either group or individual therapy. Supervision by a senior psychopharmacologist permits the use of sophisticated, state-of-the-art medications and combinations of medications to treat a wide range of mental disorders.

 

Group Therapy

The AOPC offers both short- and long-term groups, which meet on a regularly scheduled basis. These include crisis support groups for patients with acute psychosocial stressors; cognitive-behavioral therapy (CBT) oriented groups for patients with mood and anxiety disorders as well as psychodynamically oriented groups.

 

Individual Short-term Therapy

Short-term treatments include cognitive-behavioral therapy (CBT), Interpersonal Therapy (IPT), brief psychodynamic treatments and supportive treatment. Treatment selections here as with the other modalities are based on well-established clinical indications, such as the use of IPT for depression and CBT treatment of panic disorder, post-traumatic stress disorder and some eating disorders.

 

Long-term Treatment

In selected cases, long-term psychodynamic treatment is provided by psychiatric residents and by psychology interns/fellows as part of their academic training. Decisions about which patients might benefit from long-term treatment are made by the Access Center assessment team.

 

Family and Couples Treatment

Although the AOPC does not offer family and couples treatment on a regular basis, these interventions are offered on a limited basis when clinically indicated.

A comprehensive psychiatric evaluation will be scheduled based on clinical need. Individual, family, group, and psychopharmacological services are provided to individuals 18 years of age and over. The Treatment Center at the Ackerman Institute is at the hub of Institute activities. It is the main vehicle through which the Institute serves its community, as an educational, treatment and research center for families of all classes, racial/ ethnic groups and affectional preferences with a broad spectrum of problems. Approximately 11,000 patient visits are recorded each year for problems including school difficulties, adolescent troubles, marital issues, domestic violence, medical illness, substance abuse, stresses of life cycle changes, and bereavement. The Treatment Center provides the setting within which the bulk of training activities occurs and is a laboratory for the development of innovative clinical treatment approaches that have placed the Ackerman at the center of the family therapy field.

CITPD offers specialized, intensive ambulatory treatment for persons diagnosed with a Personality Disorder or some of its symptoms, including, but not limited to:

Treatment

The Center offers integrated, intensive  treatment consisting of DBT, psychodynamic, cognitive and psychopharmacological approaches tailored to meet the unique and individual needs of each patient.

 

We believe that psychiatric treatment, and psychotherapy in particular, is a unique, intricate and highly personal process of self-examination and self-transformation involving a complex sequence of interactions and interventions between the therapist(s) and the patient(s). It often entails a dialectic between validation and strengthening of what is adaptive in one’s behavior and an effort to change that which is maladaptive and / or self - destructive.

As a multidimensional process, psychotherapy involves a thorough exploration of problematic patterns of behaviors, feelings, cognitions, thoughts, interactions as well as other, internal and overt, aspects of one’s functioning. It may also necessitate an in-depth exploration of unconscious and conscious aspects of current symptoms and behaviors.

The process of psychotherapeutic change typically initiates and leads to new learning and problem solving, new understanding and insight, acquisition and rehearsal of new skills and behaviors combined with a development of a new, more healthy lifestyle.

 

 

Staff

CITPD programs are staffed by an interdisciplinary team of clinical psychologists, psychiatrists, psychiatric social worker, creative arts therapist, as well as by psychiatry and medical residents, psychology interns and fellows undergoing their clinical training at St. Luke’s-Roosevelt Hospital Department of Psychiatry.

            

Admission Criteria

Admission is based on a comprehensive assessment and review by CITPD multidisciplinary treatment team using the following criteria:

 

-principal or secondary DSM-IV diagnosis of a Personality Disorder

-ability and willingness to participate in intensive, group-based treatment

-commitment to 5 days per week attendance and 6 month long treatment cycle

-commitment to drug-free functioning and / or enrollment in concurrent substance abuse treatment, if needed

-ability and willingness to comply with program policies and rules assuring safety of self and others

 

Program-specific criteria 

The MORNING  [AM] program [9:30-12:00]- prominent and persistent (DSM Axis I) psychiatric symptoms and serious impairments of daily functioning combined with longstanding personality-related (DSM Axis II) interpersonal problems in treatment compliance, relationships, work or school

 

The AFTERNOON [PM] program [1:00-4:00] - serious, primarily personality-related, personal & interpersonal (DSM Axis II) problems combined with another, non-psychotic, psychiatric (DSM Axis I) syndrome. Substantial work and / or school experience.

 

The EVENING (IOP) program [4:00-6:30]- a wide spectrum of acute, primarily personality-related, personal and interpersonal problems (DSM Axis II).  Part-time employment or school.

 

The MODIFIED program [10:00-6:00] – meeting criteria for any of the above main tracks [AM, PM, IOP] but having difficulties engaging and/or continuing in treatment.

 The Dialectical Behavior Therapy program offers comprehensive evaluation and treatment to those whose problems include suicide attempts, self-injury, emotional instability, and impaired work or school performance. Frequently diagnosed with Borderline Personality Disorder, these individuals may also have symptoms of Post Traumatic Stress Disorder (PTSD), or they may engage in behaviors that damage the overall quality of their lives.

 

Outpatient Services for those with moderate difficulties provide individual and group psychotherapy, family support groups, medication consultation and Pharmacotherapy.

Day Treatment Services provide additional support to avoid hospitalization, as well as transition from an inpatient unit. Length of stay ranges from a few weeks to several months. Individual, group and family therapy, as well as medication management and vocational rehabilitation are offered.

Clinic Treatment - PCMH offers the full range of clinic services to both children and adults. Experienced social workers and psychologists serve as therapists and clinical supervisors, providing treatment in multiple models of care. Therapy is offered from a psychodynamic, family, cognitive, behavioral or medical model of care, based on the specific needs of the client. Specialized services include group therapy, play therapy for children, couples and family therapy, psychological testing, and medication consultation and management. The Eastside clinic also offers specialized expertise through its dedicated units, serving performing and creative artists, gay and lesbian individuals, clients with addiction recovery issues, children and adolescents. The Westside clinic offers individual, couple, and group therapy, medication management, and case management for consumers with serious mental illness.

 

Continuing Day Treatment Program (CDT) - PCMH provides a structured, time-limited intensive program to consumers with serious mental illness. This program offers a full range of mental health services, such as individual and group psychotherapy and medication management. The focus of CDT is on fostering greater independence and self-sufficiency. Using psychoeducation, the CDT promotes symptom reduction so that the consumer can begin an active reentry into the larger community, either through additional education, vocational training or volunteer efforts.

 

Continuing Day Treatment Program-MICA (CDT-MICA) - This time limited, cognitively based program provides additional interventions that address the needs of consumers who have problems with substance abuse. Using a four level system that increases self sufficiency and promotes reintegration into the larger community, MICA stresses relapse prevention. Consumers generally spend between six and eighteen months in the program, depending upon their individual needs.

 

Intensive Psychiatric Rehabilitation Treatment (IPRT) - IPRT provides a time limited, highly focused intervention to assist consumers to achieve a more desired functioning in the area of work, education or housing. The program is flexible, providing up to a maximum of four days per week, to allow a tailored schedule that facilitates the consumer's reintegration into more routine aspects of daily life. For example, consumers are likely to reduce the number of hours in program as they return to school or begin a volunteer job. Most consumers graduate from IPRT in six to twelve months.

 

Assertive Community Treatment (ACT) - The Assertive Community Treatment Program provides on-going, community based psychiatric services to people who have histories of mental illness, multiple in-patient admissions, and difficulties obtaining or maintaining treatment in out patient treatment programs. The ACT Team conducts treatment needs assessments, which includes psychiatric services and social services. The Team makes home visits, referrals to other services as needed and appropriate, and coordination of services. The ACT Program is not time limited, but it is limited to residents of Sunset Park, Red Hook, Park Slope, and areas of Bay Ridge in Brooklyn.
